What is Menopause?

The exact meaning of the word “menopause” can vary according to different sources. For the purposes of this website, “menopause” shall be defined as the time in a woman’s life when she has not had a menstrual period for at least twelve consecutive months. Many women find menopause to be a relief from a majority of the discomforts they experience during “peri-menopause”, or the period of transition from regular, ovulatory menstrual cycles to the completion of menopause. Using these definitions, the term “post-menopause” becomes redundant, as “menopause” will be used to describe the end of menstruation until the end of life. Menopause does not have to be seen as the beginning of old-age or the end of child-bearing years. In Traditional Chinese Medicine (TCM) theory, menopause is actually the body’s natural way to slow down the aging process by returning balance and nourishment to our bodies in our later years.

Why is Peri-Menopause causing me so much discomfort?

There are different ways of looking at the causes of hot flashes, night sweats, mood changes, insomnia, heavy menstrual bleeding, dryness, bladder changes, or other complaints that some women have during this time in their lives. Current medical science is examining how hormones such as estrogen, progesterone, and others are fluctuating and changing during peri-menopause, and whether they need to be supplemented to reproductive or sub-reproductive levels to alleviate the symptoms some women experience. Traditional Chinese Medicine sees menopause as a natural part of a woman’s life, and the discomforts of peri-menopause are seen as a reflection of her current state of Yin, Yang, Qi, and Blood, and especially how well her energy and blood are circulating in her body. Treatment of the symptoms of peri-menopause in TCM is unique to each woman, and involves the integration of diet, exercise, and lifestyle changes, together with acupuncture and Chinese herbal medicine as needed, to balance the body and ensure optimal circulation of energy and blood.

How long will my Peri-Menopause symptoms last?

The number, frequency, and duration of the discomforts of peri-menopause that a woman will experience are as numerous and varied as the women who display them. A majority of women have a relatively smooth transition into their menopausal years, with perhaps some minor periods of difficulty sleeping, or perhaps some heavier menses. But there is a minority of women who experience some extreme discomforts that can last many years, perhaps even into the menopausal years. For some of these women, the goal may be to reduce symptoms to a more manageable level. Which treatment a woman will choose to alleviate her peri-menopausal discomfort, and which treatment will actually work best for her, can also vary greatly from woman to woman. With a better understanding of what is happening within their own bodies, from both a biomedical and Traditional Chinese Medicine philosophy, it is hoped that women will be able to make good choices about their own health care.

Menopause is the time in a woman’s life when the function of the ovaries ceases. The ovary, or female gonad, is one of a pair of reproductive glands in women. They are located in the pelvis, one on each side of the uterus. Each ovary is about the size and shape of an almond. The ovaries produce eggs (ova) and female hormones such as estrogen. During each monthly menstrual cycle, an egg is released from one ovary. The egg travels from the ovary through a fallopian tube to the uterus.

The ovaries are the main source of female hormones, which control the development of female body characteristics such as the breasts, body shape, and body hair. The hormones also regulate the menstrual cycle and pregnancy. Estrogens also protect the bone. Therefore a woman can develop osteoporosis (thinning of bone) later in life when her ovaries do not produce adequate estrogen.

Menopause does not occur overnight, but rather is a gradual process of transition. This transition period (known as perimenopause) is different for each woman. Scientists are still trying to identify all the factors that initiate and influence this transition. Women in perimenopause transition typically experience abnormal vaginal bleeding such as erratic periods or abnormal bleeding patterns. Eventually a woman’s periods will completely stop as she completes this transition into menopause.

The average age of onset of menopause process is 51 years old. But there is no single method to predict when a woman will enter menopause. The age at which a woman starts having menstrual periods is also not related to the age of menopause onset. As a rough “rule of thumb” women tend to undergo menopause at an age similar to that of their mothers.

Surgical Menopause

Some women have their ovaries removed (bilateral oophorectomy) prior to menopause for medical reasons. This surgery causes a sudden change in estrogen level, rather than the gradual changes of natural menopause. Thus, women undergoing surgically induced menopause usually experience more abrupt and often more severe hot flash symptoms.

When does a woman know she is in menopause?

A woman is in menopause if she has had no menstrual periods (menses) for 12 months and has no other medical reason for her menses to stop. That means she has to be evaluated by her doctor to exclude other medical causes of missed menses.

Are hormone levels or other blood tests helpful in detecting menopause?

Because hormone levels may fluctuate greatly in an individual woman, even from one day to the next, hormone levels are not a reliable indicator for diagnosing menopause. Even if levels are low one day, they may be high the next day in the same woman. There is no single blood test that reliably predicts when a woman is going through menopause, or menopausal transition. Therefore there is currently no proven role for blood testing regarding menopause except for tests to exclude medical causes of erratic menstrual periods other than menopause. The only way to diagnose menopause is to observe lack of menstrual periods for 12 months in a woman in the expected age range.

Definitions

Menopause is defined as absence of menstrual periods for 12 months. The menopause transition starts with varying menstrual cycle length and ends with the last menstrual period.

Perimenopause means “around the time of menopause.” It is not officially a medical term, but is sometimes used to explain certain aspects of the menopause transition in lay terms.

Postmenopause encompasses the entire period of time that comes after the last menstrual period.
