If you do not currently have diabetes you may be asking yourself whether you are at risk of developing the condition? Well, there is an easy way to finding out.

One of the easiest ways to check if your belly fat levels put you at risk of diabetes is to measure the circumference of your waist at the top of your hipbones.

According to the American Heart Association and the National Heart, Lung, and Blood Institute if your waistline measures more than 35 inches as a woman and 40 inches in a man then you are at risk of developing obesity-related health problems [2].

Potential Fat Burning Ingredients

Before we look at some potential fat burning ingredients and diet pills for diabetes it is worth looking to see just what fat burners are.

“The term ‘fat burner’ is used to describe nutrition supplements that are claimed to acutely increase fat metabolism or energy expenditure, impair fat absorption, increase weight loss, increase fat oxidation during exercise.”

A fat burner is basically a supplement that is claimed to help reduce body fat, quicker than through diet and exercise alone [3].

To “burn fat” whichever supplement you choose will have to contain an ingredient or ingredients that can firstly break down the stored fat so that it can then be burnt for energy.

Typically a fat burner would work in one of two ways:

  1. Increasing the rate that the fat is oxidized (the technical term for “burned”)
  2. Increasing the amount of energy expended. Usually through thermogenesis, which is when your internal body temperature increases

There are numerous ingredients that can cause these reactions, lets look at a few of them before seeing if any would be suitable for use if you are diabetic.

1. Green Tea

Green tea contains various antioxidants and plant compounds that offer numerous health benefits.

The antioxidants found in green tea are called catechins, and the most important one is epigallocatechin gallate (EGCG).

This particular catechin helps to inhibit the enzyme that breaks down the hormone norepinephrine [4]. When this occurs the amount of norepinephrine increases, which results in the breakdown of fat cells [5]. This broken-down fat can then be used as energy.

What is most impressive is that research has found that EGCG can help increase your metabolic rate, even when you are resting [6]. This means you are able to burn more calories without additional exercise.

Green tea is also a source of caffeine, which can also aid your fat loss efforts. It may not be the best for those suffering from diabetes, however, for reasons we will discuss soon. It may be a better idea to use a decaffeinated option, or to look at supplementation with limited caffeine.

2. Caffeine


Caffeine is probably one of the most popular weight loss ingredients used in a large number of supplements.

It is a natural stimulant and has been proven to stimulate the central nervous system, causing an increase in your metabolism, an increase in adrenaline, and an increase in fat mobilization [7].

It has been shown that consuming 300 mg of caffeine daily could result in an extra 79 calories being burnt every day [8]. While this may not seem like much, it will certainly mount up over time.

Research has found that drinking coffee could help reduce the risk of developing type 2 diabetes [9]. However, it is advised that you avoid supplementing with isolated caffeine as it could result in an increase in blood glucose levels, this is because it may impair glucose disposal [10].

3. Yohimbine


Yohimbine works in a similar fashion as caffeine. It can increase the amount of adrenaline in your body and both, directly and indirectly, influences fat oxidation.

This natural stimulant can act on the receptors that will normally suppress fat oxidation and will suppress these receptors. This allows the fat to be oxidized (broken down) [11].

The increase in adrenaline has also been shown to stimulate fat burning [12].

There are a few reasons why yohimbine may not be the best option for you. Not only is it highly stimulatory, resulting in problems including a raised heart rate, this ingredient may interact with insulin and other diabetes medication, resulting in low blood sugar levels.

4. Bitter Orange Extract

Bitter orange extract contains a compound known as p-synephrine that can increase your resting metabolic rate without any changes to your diet or exercise regime [13].

It works in a similar fashion as ephedrine, but unlike this ingredient, it is both milder and is legal to both buy and use. When taken, the bitter orange extract will increase adrenaline and dopamine production, which will cause an increase in your metabolic rate and ability to break down fat [14].

This particular ingredient may be suitable for those with diabetes, however, there is some evidence to suggest that using bitter orange extract may affect your blood sugar levels. It is advised that if you do decide to use it that you use caution and monitor your blood sugar levels closely.

5. Forskolin


Forskolin is the active compound found in the coleus forskohlii tropical plant. It has been used for centuries in traditional medicine.

Supplementing with this compound could lead to an increase in cyclic adenosine monophosphate (cAMP), which can help stimulate the release of stored fat from fat cells [15].

Research has also found that forskolin can promote an increase in testosterone production. This offers numerous benefits, including an increase in lean muscle mass [16], which can increase your resting metabolic rate.

Forskolin may help treat diabetes with research [17] showing that it could enhance:

“Glucose-mediated stimulus that induces beta cells to release insulin.”

In simple terms, forskolin can stimulate the beta cells found in your pancreas, causing the organ to produce more insulin. Insulin is a hormone that allows your body to use sugar (glucose) from the carbohydrates you eat. This stops your blood sugar levels from getting too high, or too low.

6. Green Coffee Bean Extract


Green coffee beans are simply normal coffee beans that have not been roasted. They are full of chlorogenic acid that offers numerous health benefits.

Despite being found in numerous fat burners there is little proof to show that it has any fat loss benefits in humans. 

According to research green coffee bean extract could be of benefit to those with type 2 diabetes, with a drop in fasting blood glucose levels [18].

7. Ephedrine and ECA


Ephedrine is a fat-burning compound derived from the Chinese herb, Ma Huang. It is a stimulant that can break down stored fat and can cause an increase in your metabolic rate, even without exercise [19].

When mixed with caffeine and aspirin an effective fat burning compound known as ECA is created.

Even though both ephedrine and ECA have been shown to burn fat, they are not recommended due to serious side effects associated with its use.

8. Clenbuterol


Clenbuterol is a synthetic drug that is illegal in many countries across the globe. This has not stopped it from becoming a popular way of burning fat.

Research has found that this drug could cause a process called repartitioning to occur. This is when muscle growth replaces the fat lost [20].

Potential side effects of Clen include:

  • Increased heart rate
  • Rapid breathing
  • Heart palpitations
  • Chest pain
  • Tremors
  • Anxiety
  • Electrolyte imbalance

It is for this reason that we cannot recommend clenbuterol, whether you are diabetic or not.

Are Any Of These Ingredients Safe For Diabetics?

Of the weight loss ingredients listed above, only 3 would be suitable for use by diabetics. These ingredients would be bitter orange extract, forskolin, and green coffee bean extract.

Both caffeine and green coffee bean extract may be ok, but you should limit your dosage and keep an eye on your blood sugar levels when using a supplement with these ingredients.

Prescription Drugs Approved For Weight Loss

Now, let’s look at some of the prescription drugs and whether they will be suitable for diabetics.

Prescription Weight Loss Drugs Available In The USA

There are four weight-loss drugs approved by the U.S. Food and Drug Administration (FDA) for prescription in the USA. These are Bupropion-naltrexone (Contrave), Liraglutide (Saxenda), Orlistat (Xenical), and Phentermine-topiramate (Qsymia).

Only one of these prescription drugs could be classed as a fat burner, and that is Qsymia because it is made using phentermine that has similar chemical properties as the stimulant amphetamine.

Phentermine works in 3 ways:

  1. Phentermine stimulates the hypothalamus portion of your brain, which causes a release of the neurotransmitter norepinephrine. This chemical messenger signals a fight or flight response that reduces hunger.
  2. Phentermine also causes a release of epinephrine or adrenaline that will cause your body to break down stored fat.
  3. Phentermine will also release both serotonin and dopamine, which acts as an appetite suppressant.

While side effects of Qsymia are rare there are some side effects that you should be made aware of:

  • Dry mouth
  • Sleeping problems
  • Dizziness
  • Heart palpitations
  • Flushing of your skin
  • Fatigue
  • Constipation
  • Irritability

Another weight-loss drug available in the USA is Contrave, which is a combination of two medicines; Bupropion and Naltrexone that when combined work on two areas of your brain (the hunger center and the reward system). This helps to reduce hunger and to help control any cravings you may have.

Research has found that using Contrave could help you to lose up to 4 times more weight than using just diet and exercise [21].

Contrave is not without its problems though as there are some side effects associated with its use:

  • Constipation
  • Diarrhea
  • Dizziness
  • Dry mouth
  • Headaches
  • Increased blood pressure and heart rate
  • Insomnia
  • Liver damage
  • Nausea
  • Vomiting

Prescription weight-loss drugs are not cheap with Qsymia costing $98 a month, and Contrave costing $99 per month. Incredibly Xenical will cost $725 for a 90 capsule supply, with Saxenda costing $1,364 for a supply of 15 milliliters.

Prescription Weight Loss Drugs Available In The UK

In the UK there is only one weight loss drug available for prescription. This one is Orlistat, which is not a fat burner.

Orlistat works by preventing around one-third of the fat from the food you eat from being absorbed. This undigested fat is therefore passed out with your feces.

While this will prevent further weight gain, it will not necessarily help your weight loss efforts. You will need to make changes to both your diet and exercise regime to start seeing any weight loss results.

Orlistat will only be prescribed if you meet certain criteria in the UK, and will only be recommended if you have made an effort to lose weight by making changes to your lifestyle.

To be prescribed you will also:

  • Need to have a body mass index (BMI) of 28 or more, and have another weight-related health condition. For example, high blood pressure or type 2 diabetes.
  • Have a BMI of over 30

For best results, you must combine Orlistat with a healthy diet and other weight-loss strategies, for example increasing your activity levels.

Orlistat is not without its problems and has some common side effects, these include:

  • Oily feces
  • Frequents trips to the toilet
  • Oily discharge from your rectum
  • Flatulence (wind)
  • Stomach pain
  • Headaches
  • Upper respiratory tract infections

In the UK there is no charge for Orlistat if you are prescribed by your doctor.

Prescription Weight Loss Drugs Available In Canada

In Canada, there are 2 prescription weight loss drugs available. They are Orlistat and Liraglutide, neither of which are fat burners.

Liraglutide is a prescription weight loss drug that is injected under the skin using a pre-filled, multi-dose pen. This drug activates that GLP-1 receptors found in your brain.

When activated these receptors will delay the movement of food into your intestines. It can also affect your feelings of fullness after a meal and will help to reduce your appetite. A reduction in your appetite will lead to fewer calories being consumed and ultimately weight loss being experienced.

Side effects of Liraglutide include:

  • Nausea, vomiting, and diarrhea
  • Abdominal pain
  • Indigestion
  • Constipation
  • Heartburn
  • Flatulence
  • Pain, redness, and irritation at the site of the injection

In the past, another weight loss drug (Sibutramine) was available for prescription in Canada and the USA. It has now been discontinued over safety concerns.

Sibutramine, which is also known as Meridia works by altering the neurotransmitters found within your brain. These neurotransmitters are chemicals that are produced and released to help aid the communication between nerves.

Sibutramine has been shown to block the reuptake of the neurotransmitters dopamine, norepinephrine, and serotonin, this will have an effect on nerve function and interaction. In basic terms, this weight loss drug could help reduce energy intake (calories) while also helping to increase your resting metabolic rate [22].

Despite its proven fat-burning benefits, it became apparent that using this drug could result in some pretty serious side effects, including an increased risk of heart attack and stroke [23].

Are Any Of These Prescription Drugs Suitable For Diabetics?

Of the above-mentioned weight-loss drugs, Contrave can be used by diabetics. However, you must measure your blood glucose levels prior to use. You may need to decrease the dosage of any anti-diabetic medication that is non-glucose dependent to reduce the risk of developing hypoglycemia.

Hypoglycemia is a condition that occurs when your blood sugar levels drop lower than normal.

Saxenda is not approved for use for those with type 2 diabetes. It can lower blood sugar levels, so it is recommended that it is not used alongside any other GLP-1 receptor agonist or insulin.

Both Xenical and Qsymia can also be used by those with type 2 diabetes, and are often the two that are most often recommended for those who are obese and have the condition.

It is recommended that if you have been prescribed Qsymia and if you have type 2 diabetes that you have a discussion with your doctor about any potential side effects. There is a risk that you may encounter problems such as impaired concentration, an increase in blood pressure and heart rate, as well as insomnia.

Can Fat Burners Cause Diabetes?

Both of these have been shown to reduce A1c and weight loss, although it is worth noting that neither drug has currently been approved by the FDA.

What Drugs Increase Insulin Sensitivity?

The two drugs used to increase insulin sensitivity are Metformin and Thiazolidinediones.

Final Thoughts: What Are The Best Fat Burners For Diabetics?

There are a few options available if you are overweight, diabetic, and looking to lose weight.

Some countries offer prescription weight loss drugs, such as Contrave, Xenical, and Qsymia. The problem is that these drugs can be expensive depending on where you live.

Your best bet would be to look at your lifestyle choices and to try to eat a little healthier and get regular exercise. You could also try supplementing with bitter orange, forskolin, and green coffee bean extract.
